Americans with Disabilities Act (ADA)

Lincolnway Special Recreation Association advocates for full participation under the Americans with Disabilities Act (ADA). Every attempt at reasonable accommodation will be made so that individuals may participate.

Required Forms

In order for LWSRA staff members to deliver safe and fun programs for all, it is critical to have current information on each participant. An annual information form is required when registering and can be completed in the CommunityPass system.

Resident versus Non-Resident Fees

Resident Fee: Residents are those whose residential property taxes are paid to either the Frankfort, Frankfort Square, Manhattan, Mokena, New Lenox, Peotone or Wilmington Park District.

Special Agreements:
*Village of Monee: Households that are within Village of Monee boundaries will be allowed to register at in-district rates.
*Homer Township: Homer Township offers assistance to its residents by covering 66% of the additional out-of-town fee portion. Registrants are responsible for the remaining 34% of the out-of-town fees charged in addition to the resident fee (subject to seasonal limits and availability).

Non-Resident: Out-of-town fees are 25% to 50% greater than resident rates. (percentage determined on a program by program basis by the LWSRA Executive Director.)
**Non-Resident Exceptions: Participants who are enrolled in classes within Lincoln-Way High School District 210 or Peotone CUSD 207U shall be able to attend LWSRA programs held at Lincoln-Way High School at an in-district rate.
**Verification required prior to registration. Not eligible for scholarships,
door-to-door transportation or SSSRA cooperative agreement.

*Fee eligibility is subject to residency verification requirements as requested by LWSRA.

Payment Policy

Fees may be paid in full at time of registration for LWSRA programs, events
and tournaments. If a payment plan option is available for a specific program, the deposit amount (25% to 50%) must be made at time of registration.

Seasonal payment schedules will be created and payments must be made by the designated date each month. If a payment is not made on time, a late fee of $20.00 will be assessed and the account marked delinquent. New registrations will not be accepted until all outstanding fees are paid for the entire household.

LWSRA reserves the right to immediately deny participation to any individual who becomes delinquent until paid in full.

For Non-sufficient funds checks or e-check payments, a $35.00 fee will be assessed to your account.

Atlantoaxial Instability Release (AAI)

Individuals with Down syndrome are at risk of having a condition known AAI. This condition allows for increased laxity of ligaments involving the top bones of the neck. This results in the abnormally increased movement between the first and second vertebrae and may lead to pressure on the spinal cord which can lead to injury. Certain programs require this release.

Application for Participation in Special Olympics (Med App)

In order to compete in Special Olympics Illinois competition, participants must have completed all forms required by Special Olympics. All forms may be filled out then submitted directly to Special Olympics Illinois.

Current Required Forms:

  • Athlete Consent Form (once submitted to Special Olympics Illinois, this form is valid for life)
  • Athlete Medical Form (this form is valid for three (3) years from the date of the doctor’s exam)
