Participant Introduction Form

Thank you for your interest in LWSRA programs! New Participants (and those who last participated prior to 2020) are asked to complete this short introduction form. 

Once submitted, you will be taken to our intake meeting form where you may schedule your welcome meeting. This provides an opportunity to gain a better understanding of what services might benefit you. Please plan to have the individual(s) that will be participating in programs attend the in-person meeting! We’ll also give you a tour of the facility, discuss program options and help set-up your CommunityPass account for access to online registration. 

Please note: We are receiving a large number of new participant inquiries each week (which is a great thing!). Our staff are working diligently to meet with each new family, however this process may take 2-3 weeks depending on availability. We appreciate your patience!  

If you participated in 2021 or more recently, you should already have a CommunityPass account and do NOT need to complete the form below. Please contact us for help with your log-in information.
