LWSRA staff members facilitate interactive activities for special services students from area school districts during scheduled visits to the Lincolnway Special Recreation Center in New Lenox.

A Certificate of Insurance provided by the School District is required (in lieu of individual waivers). You may contact the LWSRA office to see if one is already on file for your district.
The above named agrees to abide by the rules set forth by the Lincolnway Special Recreation Association (LWSRA) and agree to save and hold harmless, and reimburse LWSRA, with respect to any claims, suits, attorney fees, and any other expenses which may arise due to personal injury or property damage suffered or incurred in connection with or incident to the use of the Lincolnway Special Recreation facilities by the undersigned. I also understand that the fee paid by us is to reimburse LWSRA for the additional time required of staff and maintenance personnel in connection with the use of the facilities.



I (we) agree that I (we) will be responsible for the conduct of the group which has been granted the use of the Lincolnway Special Recreation Facilities, and shall conform to the official operation policies relating to buildings and properties established by LWSRA. I (we) further agree to reimburse LWSRA in full for the the fee as agreed upon for the above mentioned field trip in addition to any loss due to the breakage, defacing of property, or other damage to the premises.
